No. While we accept most major insurance plans, including Medicare and Medicaid, we are a Federally Qualified Health Center (FQHC) dedicated to serving everyone. If you do not have insurance, we offer a Sliding Fee Discount Program based on your household income and family size to ensure healthcare remains affordable.
To ensure a smooth check-in process, please bring:
- A valid photo ID and your insurance card (if applicable).
- All current medications in their original containers (or a detailed list with dosages).
- Proof of income (such as a recent pay stub) if you wish to apply for the Sliding Fee Scale.
- Co-payment or nominal fee as required by your plan or the sliding scale.

This "whole-person" approach ensures that both your physical and emotional health are addressed in a single visit.
Yes. While we provide primary care, we coordinate referrals for specialized needs such as cardiology, orthopedics, or dermatology. Our staff will handle the scheduling and provide you with all necessary instructions for your specialist visit, ensuring your medical records are shared securely for continuity of care.
The easiest way to request a refill is through our Patient Portal or by calling your clinic directly. Please allow 48–72 hours for refill requests to be processed. We also offer access to affordable medications through our discounted pharmacy programs for eligible patients.
